Documents to file a claim

 

  • Copy of the Insurance Policy.
  • Declaration letter fill out by the adjuster, where you describe the accident and damages and the approximately amount to claim.
  • Copy of Identification and proof of address of the insured.
  • Copy of Identification and proof of address of 3rd party.
  • Claim letter from 3rd party to the insured asking to pay for the damage.
  • Copy of the Police report with ratification.
  • Quotation to repair the damages, that is provide for the workshop that the insurance company has an agreement with.
  • Copy of driver licence.
  • Identification form (article 492) fill out and sign by the person who is going to receive the indemnification.

  • Copy of Insurance Policy

    *Documentation proving ownership of the vehicle:
    • Original Title or Invoice
    • Pediment of Importation (if the vehicle is imported)
    • Payment receipts for Tenencia Tax
    • Registration card
    • Keys of the vehicle (if you have it).

  • Copy of Insurance Policy
  • Claim letter to the Insurance Company where you describe the incident and the approximately amount to claim.
  • Copy of Identification and proof of address of the insured.
  • Copy of the report to the Minsterio Público where you report the vehicle stolen and ratification.
  • Copy of the report to the PFP (Federal Police) and ratification.
  • Identification form (article 492) fill out and sign by the person who is going to receive the indemnification.

  • Call the 800 number on your policy and ask for the service you need they will send the provider and you only need to show copy of your policy.

    In case of Reimbursement:
  • Copy of Insurance Policy.
  • Claim letter to the Insurance Company where you describe the accident and the service you paid for (tow, mechanical, battery service etc).
  • Copy of Identification and proof of address of the insured.
  • Documents, Invoice or receipts that shows the service provided, under the name of the provider the will indicate when you call to do the report.
  • Copy of Insurance Policy.
  • Copy of the identification  and proof of address of the insured
  • Take your vehicle to the local provider the insurance executive indicates
Adjuster will provide a medical service pass at the moment of the accident to get immediate assistance.

  • Copy of Insurance Policy.
  • Copy of Identification and proof of address of the insured.
  • Medical records (diagnosis, medical treatment description and medical report)
  • Invoice or proof of payment of the hospitalization under patient´s name.
  • If there are medicine expenses: Doctor´s prescription and invoice under patient´s name.
  • If X Rays or Lab studies: Medical order for the studies, results and/or interpretation of x rays or studies, invoice under patient´s name.
  • Identification form (article 492) fill out and sign by the person who is going to receive the indemnification.
